THE NLMM APPROACH

At NLMM, we believe in providing care “beyond the dispensary”: our unique Diabetes Medication Management Program is a dynamic method that facilitates ongoing engagement with our clients to help them achieve their wellness goals. Our systematic approach consists of an initial consultation, and ongoing follow-up consultations to ensure continuity of care.

Initial Consultation: 30 min

The initial consultation allows the NLMM team to establish a baseline, and to discuss any questions or concerns you may have in regards to your condition and medications in general. Specifically, the initial consultation consists of the following:

  1. Medication review and optimization: Each of your medications will be assessed in the areas of necessity, efficacy, and safety; any drug therapy problems will be dealt with directly in correspondence with other members of your healthcare team. Proper use of medication devices including insulin injection and blood glucose monitors will also be discussed.

  2. Medication compliance optimization: NLMM’s partner pharmacies provide a wide range of compliance mechanisms to seamlessly integrate medication use with your lifestyle so you can work and play without worry! Services to improve compliance include daily dispensing and delivery straight to your home, blister packing, and specialized, pouched medication packaging.

  3. Lifestyle and non-pharmacological interventions: Diet and exercise play important roles in the long-term management of metabolic conditions like diabetes. Lifestyle measures and wellness strategies will be discussed and implemented.

  4. Establish wellness goals: NLMM pharmacists recognize that no two individuals are the same – therefore, we will customize a set of therapeutic and lifestyle goals specific to you.

  5. Establish monitoring plan: To achieve wellness goals, a monitoring plan suitable for your lifestyle will be created and regularly reviewed during follow-ups.

Follow-up Consultation: 15 min

NLMM follow up sessions serve to assess the effectiveness of wellness strategies and the monitoring plan established in the initial consultation. This presents an opportunity to address any questions or concerns, and as appropriate, modifications to the wellness goals and monitoring plan may be completed.

We understand the value of your time, and for NLMM to be truly effective, consultations must adapt to your schedule and lifestyle; depending on the monitoring parameters established, follow up sessions may utilize communication platforms including phone, email, or on-site at a partner pharmacy.

WHAT IS DIABETES?

Diabetes Mellitus is a chronic metabolic disturbance characterized by the body’s inability to produce sufficient insulin (Type-1) and/or inability to respond to insulin (Type-2), with both circumstances leading to elevated blood glucose levels (hyperglycemia). Prolonged hyperglycemia can lead to serious complications such as retinal damage, kidney damage, neuropathy, and severe vascular consequences including heart attack and stroke.

As per the Canadian Diabetes Association, it is estimated that 2 million Canadians are affected by diabetes, a number set to increase to 3.7 million by 2019. Altogether, the combined total of Canadians with prediabetes and diabetes currently sits at an estimated 11 million. Progression of the disease, however, can be decelerated by the combined efforts of effective, timely drug therapy and lifestyle interventions. Our goal at NLMM is to utilise this principle to the fullest for every one of our clients.
